The Association provides in writing with all the information required, Society Name, Society Address, Gym Address, Head Trainer, Trainer (Instructor), Address, Tel Ph. No. , Fax Number, internet Email, Web, Society President, Society Secretory, Stamp & Signature & Date with supportive documents & Submit this Form by click on submit button and after it pay fee online.
